Whipped this up so I can keep track of my OC's transforms, and thought I'd offer a version here. It's made to match up with the ID Sheet I also offer.

You'll need to Download the "Freebooter" font from here :
[link] for the title, otherwise you'll get an error. (There should be lots of tutorials on how to install fonts out there - I won't do it here because of how many different systems there are out there).

Will this work in SAI or GIMP or <insert program here>?- NO. Because of the layer styles on some of the text, this will only work in Photoshop (far as I know).
How do I replace the name?-Just use the text tool in Photoshop, on the layer that's already there (just erase the text that's there with backspace and replace it with your own name).
Can I remove/add things?Of course, so long as you don't then try to offer this as your own template - feel free to make whatever changes to the template that you need.
Can you make a version for (insert program)?No. I only know how to use Photoshop and SAI - and SAI doesn't have text support so it's useless for this kind of thing.
Why didn't you include Charmix?The original Charmix was useless in my opinion, so I didn't make it for my OC's - hence, I didn't include it here. Feel free to add another box for it if you want. Also, the first box can be changed, according to whether you call the first transformation Winx or Charmix.
Can you make it in <insert colour here>?No, but feel free to change any colours you want.
Easiest method: To change the color of something, select the layer that has the item whose color you want to change. Then go to the Image tab on the toolbar of Photoshop, then Adjustments -> Hue/Saturation and select your desired color using the color slider.
